Building Use Forms

Church members should fill out the "Internal Facilities Use Request Form" and have it approved by a staff member for any activity that requires reserving the use of the building or the church van. It should also be filled out to inform the office of events that should be added to the church calendar which occur outside the church building.

Groups outside the church wishing to meet at the church should fill out the "Building Use Request Form" and any pertinent riders. If children are involved in the group, read the "Child Protection Policy" and submit the Child Protection Rider with your form. If you are requesting the use of the kitchen, the organ or a piano, fill out the necessary form for that as well. The application will be reviewed by the Trustees at their monthly meeting which occurs on the 3rd Monday of most months.
